# Add Form

**POST** `/v1/forms`

Creates or submits: Add Form.

Create a new form with attribute configuration (Admin Only).

**Body Parameters**

| `metadata` | optional | `object` | Arbitrary JSON metadata. |
| `attributes` | optional | `array<object>` | Form attributes list with `attribute_id`, `required`, and `order_by`. |
